How do we implement this?

For each chain approach there is a network consultation. This includes partners who help with implementation. This network meets four times a year. The goal is better collaboration and thus contributing to a healthy and happy life for residents.

For each chain approach there is a support route:

The green route is for self-reliant residents.

The orange route is for vulnerable residents.

The red route is for very vulnerable residents.
